Job summary

Indiana University Health in Fort Wayne seeks a relationship-driven registered dietitian to join IU Health Home Care as a Dietitian Referral Liaison. The role blends clinical nutrition expertise with building referral networks and coordinating home enteral nutrition referrals.

Travel across the Fort Wayne area is involved to support hospital teams, physicians, dietitians, case managers, patients, and caregivers, enabling safe transitions from hospital to home.

Qualifications

• Registered dietitian with clinical enteral nutrition experience.
• Strong relationship-building and communication skills.
• Ability to educate patients and caregivers on tube feeding and pumps.

Responsibilities

• Coordinate home enteral nutrition referrals for Fort Wayne area.
• Provide tube-feeding and pump education to patients and caregivers.
• Troubleshoot clinical needs and support hospital teams, physicians, and case managers.
• Build and maintain referral relationships to grow home care referrals.
