Must-Have:
Minimum of a Master’s degree in Nutrition, Dietetics or related field.Completion of required coursework, Dietetic Internship, and successful passing of the CDR exam, or a Bachelor’s degree with completion of required coursework and Dietetic Internship if the CDR exam is passed before 01/01/24.
Description:Assesses patients' nutritional status.Develops appropriate medical nutrition therapy care plans.Provides nutrition counseling and education to patients and caregivers.Documents recommendations for treatment in the patient's electronic medical record according to facility policy.Communicates effectively with coworkers and families to ensure the appropriate nutrition care plan is implemented and followed.Participates in interdisciplinary rounds with medical, nursing, and rehab staff.Coordinates care with interdisciplinary team members.Performs all duties and responsibilities for designated inpatients and ambulatory care patients.Functions as a member of the multidisciplinary team to develop, coordinate, implement, and evaluate patients' nutritional care plans.Documents recommendations and treatment in the medical record according to facility policy.Complies with all hospital-wide and departmental policies and procedures.On-call and weekend coverage are not required.Holidays are required as scheduled.May be required to float to other locations until the specified date.
