Qualifications

• Experience conducting nutritional assessments and developing individualized nutrition care plans.
• Strong knowledge of therapeutic diets and hydration management.
• Excellent communication and collaboration with interdisciplinary teams.

Responsibilities

• Complete and oversee nutritional assessments for residents.
• Develop individualized nutrition care plans.
• Monitor residents' weights, intake, hydration status and labs.
• Provide nutrition counseling to residents and families.
• Review menus and nourishment programs for adequacy and regulatory compliance.
• Participate in care plan and quality improvement initiatives.
• Maintain documentation and nutrition-related records.
• Assist with policy review and education programs.
• Perform additional duties as assigned.
