Qualifications

• Registered by the Commission on Dietetic Registration or equivalent professional credential.
• State licensure required.
• Membership in the Academy of Nutrition and Dietetics preferred.

Responsibilities

• Assess patients' nutritional needs and dietary status.
• Develop and implement individualized dietary plans.
• Educate patients on healthy eating habits and post-discharge nutrition.
• Instruct patients on post-discharge diets and monitor progress.
• Monitor patients' nutritional status and adjust plans as needed.
• Document pertinent information in the medical record.
• Collaborate with physicians and other disciplines to optimize care.
