We are partnering with a well-established healthcare organization focused on delivering high-quality, patient-centered care to older adults in the community. This part-time Dietitian role offers the opportunity to make a meaningful impact by supporting individuals in maintaining their independence, health, and overall quality of life. In this role, you will collaborate with a multidisciplinary team to assess nutritional needs, develop personalized care plans, and provide education to patients and their families. This position includes a mix of clinical and community-based work, including occasional home visits.
Key Responsibilities
- Conduct comprehensive nutritional assessments to identify dietary needs and preferences
- Develop and implement individualized nutrition care plans
- Provide nutrition education and counseling to patients, families, and caregivers
- Support disease-specific nutrition management (diabetes, heart health, renal care, etc.)
- Promote wellness and encourage self-management through education and support
- Inspect meals for compliance with prescribed diets and quality standards
- Assist with menu planning, recipe development, and coordination with food vendors
- Collaborate with physicians, nurses, social workers, and therapists to provide integrated care
- Maintain accurate and timely documentation in the Electronic Health Record (EHR)
- Participate in team meetings, caregiver groups, and community events as needed
- Ensure compliance with all state and federal regulations, including HIPAA
Minimum Qualifications
- Active Oregon Licensed Dietitian (LD) credential
- Bachelor’s degree in Dietetics, Nutrition, Food Systems Management, or related field
- Valid CPR certification
- Valid driver’s license
Desired Qualifications
- Experience working with elderly or medically complex populations
- Background in community-based care, home health, or long-term care settings
- Familiarity with interdisciplinary team (IDT) models of care
- Strong communication skills with patients, families, and care teams
