Registered Dietitian Role Summary:

This role is responsible for assessing the nutritional status of patients and designing tailored nutrition interventions to optimize health and well‑being. The Registered Dietitian will actively communicate nutritional insights to the team, contribute to comprehensive care plans, and provide education to patients, families, and healthcare professionals. Key Duties and Responsibilities:
  • Provide high‑quality nutritional care and serve as an active member of the healthcare team.
  • Conduct comprehensive initial and periodic nutritional assessments for patients.
  • Collaborate with the team to develop and revise individualized care plans that address patients’ unique nutritional needs.
  • Deliver nutritional counseling and education to patients, family members, and caregivers to promote wellness and improved health outcomes.
  • Facilitate nutrition‑related interventions, including home‑delivered meals and dietary supplements.
  • Monitor weight, medical conditions, medication‑nutrient interactions, changes in chewing or swallowing, mood, hydration, intake, and skin integrity; report changes with recommendations to the team.
  • Support the healthcare team in understanding nutritional implications of patients’ health conditions.
  • Incorporate patients’ cultural and ethnic preferences into care planning.
  • Serve as a resource for other healthcare professionals regarding nutrition‑related care.
  • Participate in family meetings, staff meetings, training, and orientation programs.
  • Contribute to quality improvement initiatives and maintain best practices in nutritional care.
  • Follow safety guidelines, infection control protocols, and confidentiality standards.
  • Document care accurately, concisely, and comprehensively.
  • Perform other duties as assigned, including temporarily assuming Food Service Supervisor responsibilities if needed.
Required Qualifications:
  • Bachelor’s degree in Food, Nutrition, or Dietetics from an accredited institution.
  • Successful completion of the Commission on Dietetic Registration (CDR) exam.
  • Minimum of 1 year of experience working with the elderly population; training provided if necessary.
  • Valid driver’s license, reliable transportation, and required auto insurance.
  • Medically cleared with up‑to‑date immunizations for direct patient contact.
  • Ability to pass comprehensive background checks and screenings, including federal and state exclusions, education, license, and drug screening.
Desired Qualifications:
  • Minimum of 2 years clinical nutrition experience preferred.
  • Serve Safe Certification preferred.
  • Knowledge and clinical experience in medical nutrition therapy for common chronic conditions (e.g., diabetes, heart disease).
  • Strong computer literacy, including Microsoft Office Suite, electronic health records, and general office equipment.
  • Excellent communication, interpersonal, and collaboration skills, with a “people first” approach.
  • Creativity, innovation, and the ability to “think outside the box” in care planning and problem‑solving.
  • Familiarity with managed care environments and cost‑effective, quality‑focused practices.
  • Commitment to person‑centered care, advocacy, and supporting patient self‑determination.
Work Environment & Physical Requirements:
  • Work involves collaboration within a healthcare team in a clinical or adult day health setting with moderate noise and controlled temperatures.
  • Manual dexterity, hand‑eye coordination, and ability to operate equipment are required.
  • Ability to lift and carry up to 30 pounds safely.
  • Corrected vision and hearing within normal range. Effective communication with patients, staff, and external contacts is essential.
  • Exposure to typical healthcare risks, including bodily fluids, medications, communicable diseases, and moving equipment.
