Job summary

Salamanca Rehabilitation and Nursing Center in New York state is seeking a licensed dietitian to join our team for full-time, dual-facility coverage. You will assess residents’ nutrition needs, develop care plans, and collaborate with the food service team to implement therapeutic diets.

Requirements include a BS in nutrition, NY certification, and 1–3 years of direct care experience (2 years in long-term care preferred). Master’s degree is a plus.

Qualifications

• Bachelor’s degree required; NY State dietetics/nutrition certification when eligible.
• Master’s degree preferred with emphasis in nutrition.
• One to three years’ direct care experience; 1–2 years in long-term care.

Responsibilities

• Assess nutrition status including laboratory data, fluid balance and energy needs.
• Evaluate data, review and modify plans of care, and deliver nutrition care.
• Provide nutrition information through meal rounds, test trays and menu checks.
• Plan and deliver nutrition care for residents fed by enteral means.
• Prepare residents for self-management of nutrition care.
• Help develop QA and improvement processes within the facility.
• Consult with Food Service Director to develop therapeutic diets.
• Refer to other health care providers as necessary.

Experience Required

One to three years’ direct care experience with one to two years’ experience in long- term care.

Essential Functions

• Assess nutrition status, including laboratory data, fluid status, energy and protein needs ofresident.
• Evaluates and interprets data, reviews and modifies plans of care, and delivers nutrition care to residents.
• Routinely provides nutrition-related information/technical assistance by conducting meal rounds, test trays, menu checks and other services as needed.
• Accurately assesses, plans and delivers nutrition care to residents fed by enteralmeans.
• Prepare residents for self-management of nutritioncare.
• Helps develop and implement Quality Assurance and Improvement processes within thefacility.
• Consults with Food Service Director to develop appropriate therapeuticdiets.
• Makes referrals as necessary to other health careproviders.
• Completes other duties to support departmental operations as assigned orrequested.
• Participates in inspections (surveys) conducted by authorized agencies
• Provides safe resident nutritional care while demonstrating knowledge and competency in ordering nutritional interventions, and in accordance with physician/NP orders, according to established standards and policies.
• Maintains awareness of new updates/standards and incorporates them into his/her nutritional practice throughout the continuum of care.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
• Reports any change in condition of a resident to the appropriate nursingsupervisor.
• Reports any resident care issues to the appropriate nursing supervisor.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
