Job summary

Sapphire Center for Rehabilitation and Nursing is seeking a part-time Registered Dietitian in Queens, NY. Qualifications

Responsibilities

• Interviews patient and/or family for nutritional history and preferences; documents in medical records.
• Performs nutritional assessments and develops patient-specific care plans.
• Recommends therapeutic diet modifications and supplements to physicians with diet orders checked for accuracy.
• Calculates nutrient content of tube feedings and dietary intakes as ordered by physicians.
• Provides nutrition guidance and counseling to patients and families.
• Assists with menu changes and standardized recipes; oversees food service staff in safe handling and portions.
• Delivers individual and group nutrition education sessions as needed.
• Communicates nutrition information with interdisciplinary care team daily.
• Maintains knowledge of state and federal guidelines; uses EMR and menu software.

Sapphire Center for Rehabilitation and Nursing, a premier long term skilled nursing facility located in Queens, New York is seeking a part-time Registered Dietician to join its team of exceptional care professionals. The Registered Dietitian will be responsible for assessing the nutritional status of patients and recommending the appropriate nutrition therapy as it pertains to the patients’ nutritional needs. The Registered Dietitian will also be responsible in assisting the dietary staff in creating high quality meals and the delivering of a meaningful dining experience for clients in a long term skilled nursing facility.

Job Responsibility

• Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients’ medical record.
• Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration.
• Assists in recommending therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician’s order.
• Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians.
• Provides nutritional guidance and counseling to patients and their families.
• Assists with changes to patient’s menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handing and portion control.
• Prepares, presents, and conducts individual and group education classes, as needed.
• Develops and utilizes effective lines of communication regarding nutrition information within the facility by coordinating with an interdisciplinary team on a daily basis to provide quality care to residents,
• Knowledge of applicable state and federal guidelines.
• Computer literacy and proficiency with EMR software and computer-based menu systems

Job Qualification

• Bachelor's Degree required, or equivalent combination of education and related experience.
• Current Registered Dietician Certification with the NYS Department of Education.
