Job summary

Yorktown-Rehabilitation-and-Nursing-Cente seeks a qualified Dietitian to assess residents’ nutrition status, interpret data, and develop care plans in a two-facility setting in New York. The role includes diet planning, nutrition education, and collaboration with care teams to ensure high-quality resident nutrition care.

Requirements include a Bachelor’s degree in nutrition, NY licensure as a dietitian, and 1–3 years of direct care experience in long-term care.

Qualifications

• Bachelor’s degree from a regionally accredited college or university in the United States.
• Master’s degree with emphasis in nutrition preferred from an accredited program.
• Certification as a dietitian or nutrition professional in NY State once requirements met

Responsibilities

• Assess nutrition status, including laboratory data, fluid status, energy and protein needs of residents.
• Evaluate data, review and modify care plans, and deliver nutrition care to residents.
• Provide nutrition information and support through meal rounds, menu checks, and other services as needed.
• Plan and deliver nutrition care to residents fed by enteral means.
• Assist in self-management of nutrition care for residents.
• Collaborate with the Food Service Director to develop therapeutic diets.
• Refer to other health care providers when needed.
• Support department operations and participate in surveys conducted by authorized agencies.

Education

Bachelor’s degree in nutrition

Master’s degree in nutrition preferred

Essential Functions

• Assess nutrition status, including laboratory data, fluid status, energy and protein needs ofresident.
• Evaluates and interprets data, reviews and modifies plans of care, and delivers nutrition care to residents.
• Routinely provides nutrition-related information/technical assistance by conducting meal rounds, test trays, menu checks and other services as needed.
• Accurately assesses, plans and delivers nutrition care to residents fed by enteralmeans.
• Prepare residents for self-management of nutritioncare.
• Helps develop and implement Quality Assurance and Improvement processes within thefacility.
• Consults with Food Service Director to develop appropriate therapeuticdiets.
• Makes referrals as necessary to other health careproviders.
• Completes other duties to support departmental operations as assigned orrequested.
• Participates in inspections (surveys) conducted by authorized agencies
• Provides safe resident nutritional care while demonstrating knowledge and competency in ordering nutritional interventions, and in accordance with physician/NP orders, according to established standards and policies.
• Maintains awareness of new updates/standards and incorporates them into his/her nutritional practice throughout the continuum of care.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
• Reports any change in condition of a resident to the appropriate nursingsupervisor.
• Reports any resident care issues to the appropriate nursing supervisor.
• Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
• Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
