Job summary

Personal Healthcare Management LLC is seeking a Nutrition Services professional to assess and manage residents’ nutrition needs in Plattsburgh and Massena facilities.

Responsibilities include evaluating data, planning care, and delivering nutrition services, including enteral care and self-management training, while supporting quality improvement initiatives.

Qualifications

• Bachelor’s degree required in the United States from a regionally accredited college or university.
• Master’s degree with emphasis in nutrition preferred.

Responsibilities

• Assess nutrition status and plan care for residents.
• Review data, modify care plans, and deliver nutrition care to residents.
• Provide nutrition information through meal rounds, menu checks, and related services.
• Assess and deliver nutrition care for residents fed by enteral means.
• Prepare residents for self-management of nutrition care.
• Support QA and quality improvement processes within the facility.
• Consult with Food Service Director to develop therapeutic diets.
• Refer to other health-care providers as needed.

Education

Bachelor's degree in dietetics or related field (US regionally accredited)

Master's degree with emphasis in nutrition (preferred)

Shift: Full-Time

This position is a FT position to cover both Plattsburgh and Massena Facilities.

Qualifications:

1) Bachelor’s degree or higher from a regionally accredited college or university in the United States

Educational Requirements

Bachelor’s degree required; Master’s degree with emphasis in nutrition preferred from an accredited program.

Experience Required

One to three years’ direct care experience with one to two years’ experience in long- term care.

Professional Licensure and Certification Required

Certification in dietetics-nutrition in New York State

Essential Functions

1. Assess nutrition status, including laboratory data, fluid status, energy and protein needs ofresident.
2. Evaluates and interprets data, reviews and modifies plans of care, and delivers nutrition care to residents.
3. Routinely provides nutrition-related information/technical assistance by conducting meal rounds, test trays, menu checks and other services as needed.
4. Accurately assesses, plans and delivers nutrition care to residents fed by enteralmeans.
5. Prepare residents for self-management of nutritioncare.
6. Helps develop and implement Quality Assurance and Improvement processes within thefacility.
7. Consults with Food Service Director to develop appropriate therapeuticdiets.
8. Makes referrals as necessary to other health careproviders.
9. Completes other duties to support departmental operations as assigned orrequested.
10. Participates in inspections (surveys) conducted by authorized agencies
11. Provides safe resident nutritional care while demonstrating knowledge and competency in ordering nutritional interventions, and in accordance with physician/NP orders, according to established standards and policies.
12. Maintains awareness of new updates/standards and incorporates them into his/her nutritional practice throughout the continuum of care.
13. Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
14. Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their ownactions.
15. Reports any change in condition of a resident to the appropriate nursingsupervisor.
16. Reports any resident care issues to the appropriate nursing supervisor.
17. Assists co-workers as needed and maintains positive interpersonal relationships withcolleagues.
18. Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their own actions.
