Nutrition that Works, LLC is seeking a consultant dietitian to join our team for our long-term care community in Olean, NY. This opportunity is 8 hours per week with a flexible schedule and excellent pay.

Responsibilities include completing clinical nutrition assessments, documenting with the Nutrition Care Process in the EMR, collaborating with the interdisciplinary care plan team, and completing the MDS - Section K.

Qualifications

• Must be credentialed by the CDR.
• Licensed as a dietitian.
• Strong communication and client-centered approach.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF and make wound/weight-change recommendations.
