Long Term Healthcare Services in Haskell, NJ is seeking a compassionate Registered Dietitian to join our 200+ bed skilled nursing facility. You will provide high-quality nutritional care to residents and participate in interdisciplinary teams.

The role includes conducting assessments, updating care plans, meal planning, and counseling families. 5K sign-on bonus offered; competitive salary and full benefits.

Qualifications

• Registered Dietitian license (NJ) required.
• Experience in long-term care or rehabilitation preferred.
• Experience with PointClickCare EMR preferred.
• Experience with MealTracker preferred.
• Strong communication skills (verbal and written).

Responsibilities

• Deliver high-quality nutritional care to residents.
• Complete admissions, re-admissions, quarterly, annual and significant-change assessments.
• Monitor and document resident weights, weight changes, ulcers, and wound care nutrition needs.
• Manage nutrition for hemodialysis and enteral feeding residents.
• Complete MDS documentation and update care plans.
• Participate in interdisciplinary care plan meetings.
• Review meal and snack preferences, enter diet orders, and assist with menu planning.
• Conduct meal rounds and nutrition audits.
• Provide nutrition education and counseling to residents and families.
• Attend morning clinical meetings as required.

Key Responsibilities

The Registered Dietitian will be responsible for delivering high-quality nutritional care to residents, including:

• Completing new admission, re-admission, quarterly, annual, and significant change assessments

• Monitoring and documenting resident weights, weight changes, pressure ulcers, and wound care nutrition needs

• Managing nutritional care for hemodialysis and enteral feeding residents

• Completing MDS documentation

• Updating care plans and participating in interdisciplinary care plan meetings

• Reviewing and updating resident meal and snack preferences, entering diet orders, and assisting with menu planning as needed

• Conducting meal rounds and nutrition audits

• Providing nutrition education and counseling to residents and families

• Attending morning clinical meetings as required

• Maintaining strong communication with physicians, nurse practitioners, nursing staff, and the interdisciplinary care team

• Performing additional related duties as assigned

Qualifications

• Active Registered Dietitian license in the state of New Jersey required

• Long-term care or short-term rehabilitation experience preferred

• Experience with PointClickCare EMR preferred

• Experience with MealTracker preferred

• Ability to make independent clinical decisions

• Strong understanding of nutritional service regulations, standards, and best practices within long-term care settings

• Excellent verbal and written communication skills
