Job summary

Asian Health Services seeks a PACE Dietitian Manager to lead the medical nutrition program for PACE participants. You will assess nutritional needs and integrate industry best practices with the IDT to tailor interventions and care plans.

Responsibilities include meal planning at the PACE Center, nutrition education for participants and caregivers, and supervising dietetic staff while ensuring accurate documentation in the PACE EHR system.

Qualifications

  • Bachelor’s degree in Nutrition from an ADA-accredited institution.
  • Certified as a Registered Dietitian (RD) or RD Nutritionist (RDN).
  • Minimum 1-year work experience as a Dietitian.
  • Leadership and management experience.
  • Bilingual ability in Cantonese and/or Mandarin.

Responsibilities

  • Complete nutrition assessments for new and existing PACE participants as part of the IDT care planning process.
  • Develop and implement nutrition and dietary care plans with the IDT.
  • Plan and supervise food services and menus at the PACE Center.
  • Provide nutrition education to participants, caregivers, and staff.
  • Supervise nutrition staff and maintain PACE EHR documentation.

Job Summary:

The PACE Dietitian Manager is responsible for the medical nutrition program for PACE participants.

ThePACEDietitianManager is a member of the PACE InterdisciplinaryTeam(IDT)andis responsible forcompletingnutrition assessmentsfor newPACEparticipantsandnutritionreassessments on existingPACEparticipants as part of thePACEIDT care planning process.

The PACE DietitianManager is responsible forassessingparticipants' nutritionaland dietaryneedsandincorporatingindustrybest practicesto implementappropriate nutritionaland dietaryinterventionsfor PACE participants in collaboration with the IDT.

The PACE DietitianManager is responsible forplanning food servicesat the PACE Centerandensuringparticipant food preferences are consideredin PACE Center menu planning.

The PACE DietitianManager is responsible forprovidingnutritionand dieteducation to participants, caregivers,andPACE staff.

  • Participates as a PACE Interdisciplinary Team (IDT)memberandis responsible forcompleting nutrition assessments for newPACEparticipants and nutrition reassessments on existingPACEparticipants as part of thePACEIDT care planning process.
  • In collaboration with the primary care provider and IDT,developsandimplementsnutrition anddietcare plansfor participants,identifyingand consideringthemedical, physical, social,and emotional needsofPACE participants.
  • Assesses PACE participants' food security status and recommends to the IDTmeasuresto address participant food security issues throughPACEservice delivery interventions andthe care planning process.
  • In concert with the primary care provider,assessesparticipants' needsfor nutritional supplements.
  • Monitors PACE participants for changes in their health or psychosocial status requiringa nutrition reassessment.Promptly communicatespertinentchanges in participantnutritionalor dietarystatus to the IDT.
  • Attends IDT meetings and morning meetingsandreports on participants' nutritional status, including changes to individualparticipant foodintakeandnotable weight changes. Based on clinical observations or nutrition reassessments, makerecommendations to the IDT for incorporating ormodifyingtherapeutic diets andrecommendingdietary modificationstoparticipantcare plans.
  • Responsible fordailyfood services planning at the PACE Center, ensuring participants' dietaryrestrictions are enforcedandparticipantfoodpreferences considered.
  • Supervise, mentor, and evaluate nutrition staff; oversee staffing, scheduling, and performance to ensure continuity and accountability.
  • Completes andmaintainsdocumentation in the PACE EHRin accordance withthe federal PACE regulation and organizational policy.
  • Serves asa nutrition and dietsubject matter expertandcommunicatestothe IDTbest practices in nutrition management for a frail elderly population.
  • Responsible for providingnutritioneducation to participantsand caregivers. Home visits may be necessary toprovideparticipants and caregivers withnutritioneducation or nutritional assessments/reassessments.
  • Assistswith meal and snackservicein the PACE Center as needed.
  • Conductssanitation and safety checks for kitchen and kitchen equipment.
  • Supports thePACEQIprogramandparticipatesinPACEQI activitiesas assigned.
  • Bachelor’s degree in Nutrition from an ADA-accredited institution.
  • Certified as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN)
  • Minimum 1-year work experience as a Dietitian.
  • Registered member of the ADA and registered by the Commission on Dietetic Registration.
  • Leadership and management experience.
  • Bilingual ability (Cantonese and/or Mandarin) and experience providing culturally and linguistically appropriate services.
  • One year of experience working with a frail or elderly population, orin the absence of such knowledge, receiveappropriate trainingonworking with a frail or elderly population upon hiring.
  • Before working independently, meet a standardized set of competencies for theDietitianpositionestablishedby the PACE plan.
  • Ability to make home visits as appropriate and necessary.
  • Experience workingin Electronic Health Records (EHRs).
  • Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
  • Have not been convicted of criminal offensesinvolvingMedicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act.
  • Havenotbeenexcluded fromparticipatingin Medicare and Medicaid programs.
  • Have not been convicted of criminal offensespertaining tophysical, sexual, drug, or alcohol abuse.
  • BLS certified.
  • ServSafecertified
  • Certified as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN)
  • Registered member of the ADA and registered by the Commission on Dietetic Registration.
