Job summary

Asian Health Services seeks a PACE Clinic Assistant to provide general nursing care to PACE participants, perform nursing interventions under supervision, and triage medical conditions in the PACE clinic and by telephone.

You will deliver nursing education to participants, caregivers, and staff; manage care transitions, coordinate with hospital and long-term care facility case managers, and schedule referrals to specialists.

Qualifications

One year of experience with frail or elderly population or training in PACE prior to independent care.
Minimum 3 years LVN clinical experience.
Experience working in Electronic Health Records (EHR).
Bilingual ability (Cantonese and/or Mandarin) and ability to provide culturally appropriate services.

Responsibilities

Provide nursing interventions independently under supervision.
Schedule participant appointments for PACE Clinic referrals and specialists.
Enter instructions in the EHR for appointments and care plans.
Complete and maintain PACE EHR documentation per federal regulations.
Administer participant medications as directed.
Disseminate nursing orders and instructions to PACE staff.
Coordinate care transitions and communicate with hospital/long-term care case managers.
Attend IDT and morning meetings and report participant status.

Job Summary:

The PACE Clinic Assistant provides general nursing care to PACE participants, executes nursing interventions under the supervision of the PACE primary care providers and nursing leadership staff, and evaluates and triages participant medical conditions in the PACE clinic and by telephone.

The PACE Clinic Assistant provides nursing education to participants, caregivers, and PACE staff.

The PACE Clinic Assistant provides case management and care coordination during care transitions and proactively communicates with hospital and long-term care facility case managers regarding participant discharge care plans.

The PACE Clinic Assistant is responsible for scheduling participant appointments for PACE Clinic referrals made to medical specialists and ancillary care providers in the community.

The PACE Clinic Assistant supports the IDT and QI program and participates in IDT and QI activities as directed by PACE nursing leadership staff.

Under the supervision of the primary care provider and PACE nursing leadership staff, provides nursing interventions independently, manages and triages PACE participant nursing interventions in the PACE Clinic or community setting, and escalates any nursing concerns to the primary care provider and PACE nursing leadership.
Responsible for scheduling participant appointments for PACE clinic appointments and referrals to medical specialists and ancillary care providers in the community.
Responsible for entering instructions in the Electronic Health Record (EHR) for medical specialists and ancillary care provider appointments in the EHR.
Completes and maintains documentation in the PACE EHR in accordance with the federal PACE regulation and organizational policy.
Administers participant medications in the PACE Clinic as directed by the primary care provider and PACE nursing leadership staff.
Reviews, signs off, and disseminates nursing orders and instructions to appropriate PACE staff and PACE contracted personnel.
Provides case management and care coordination during care transitions and proactively communicates with hospital and long-term care facility case managers regarding participant discharge care plans.
Monitors PACE participants for changes in their health or psychosocial status. Promptly communicates pertinent changes in participant status to the IDT.
Attends IDT and morning meetings and reports on participants’ clinical status as directed by PACE nursing leadership staff.
Participates in the nursing on-call rotation to provide after-hours and weekend call coverage to PACE participants.
Supports the PACE QI program and participates in PACE QI activities as assigned.

One year of experience working with a frail or elderly population or, in the absence of such experience, receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently. Relevant experience must be within the last 3 years.
Minimum 3 years LVN clinical experience.
Experience working in Electronic Health Records (EHRs).
Bilingual ability (Cantonese and/or Mandarin) and experience providing culturally and linguistically appropriate services.
Meet a standardized set of competencies for the specific position description established by PACE before working independently.
Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs.
Have not been excluded from participating in Medicare and Medicaid programs.
Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse.
BLS certified.
Current LVN license in good standing to practice in California.
BLS certified.
