Job summary

Asian Health Services seeks a Program of All-Inclusive Care for the Elderly (PACE) Center Assistant (PCA) to join our IDT. You will provide personal care per care plans, assist with meals, and support transfers and clinic visits within the PACE Center setting.

Strong bilingual ability (Cantonese/ Mandarin) and experience with electronic health records are required; prior eldercare experience is preferred and training is provided as needed.

Qualifications

  • Bilingual Cantonese and/or Mandarin to provide culturally appropriate care.
  • Experience with frail or elderly populations or willingness to train.
  • Experience using Electronic Health Records (EHRs).
  • Must be medically cleared for communicable diseases and up-to-date vaccinations.

Responsibilities

  • Provide personal care per participant care plans (peri-care, grooming, dressing, bathing).
  • Assist with meals and monitor during PACE Center meals.
  • Support transfers and mobility with IDT guidance.
  • Accompany participants to outside medical appointments as required.
  • Monitor health and psychosocial changes and report to IDT or management.
  • Attend IDT and morning meetings and complete PCC documentation in PACEEHR.

Job Summary

The PACE Center Assistant (PCA) is a member of the PACE Interdisciplinary Team (IDT) and is responsible for providing personal care services in accordance with each PACE participant's care plan.

The PCA is responsible forprovidingpersonal care services andother PACE Centertasks as assignedby PACE Center management personnel.

  • Provides personal careservicesin accordance with individualparticipant careplans,including peri-care, grooming, dressing, showering, bathing, toileting, feeding,andambulation.
  • Assistswith food service delivery and monitoring during PACE Center meal service.
  • Implements interventions as directed byIDT disciplines,including mechanical assistancewithparticipant transfers.
  • Accompanies participants to outside medical appointments asassignedby PACE Center management.
  • Monitors PACE participants for changes in their health or psychosocial statusand promptly communicates pertinent changes in participant status to the IDTor PACE Center management.
  • Attends IDT and morning meetings asassignedby PACE Center management.
  • Completes PCC documentation in the PACEEHRincompliance with federal PACEregulationsand organizational policy.
  • Supports the PACE QI program and participates in PACE QI activities as assignedby PACE Center management.
  • High school diploma or equivalent.
  • Bilingual ability (Cantonese and/or Mandarin) and experience providing culturally and linguistically appropriate services.
  • One year of experience working with a frail or elderly population, or in the absence of such knowledge, receive appropriate training on working with a frail or elderly populationbefore providing participant care.
  • Before working independently, meet a standardized set of competencies for the PCAposition established by the PACE plan.
  • Experience working in Electronic Health Records (EHRs).
  • Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
  • Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of theSocial SecurityAct.
  • Have not been excluded from participating in Medicare and Medicaid programs.
  • Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse.
  • BLS certified
