Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Responsibilities

1. Assess patient status and implement care plans in collaboration with the interdisciplinary team.
2. Coordinate patient care and provide education.
3. Administer medications and treatments as directed.
4. Monitor and document patient responses during treatment.
5. Mobilize resources for complex cases to enhance patient recovery and participation.
6. Use assessment and diagnostic information to anticipate healthcare needs.
7. Ensure effective handoff communication.
8. Adjust staffing and develop individualized comfort measures based on patient needs.
9. Participate in performance improvement activities.
10. Perform other duties as assigned.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in dialysis preferred

Rate: $45.00 - $55.00
