Dialysis Clinic, Inc. is seeking a Dialysis Social Worker to assess and respond to the psychosocial needs of dialysis patients, improving quality of life. Fulltime, Monday–Friday, with shifts starting at 8am, serving clinics in Cullman and Lawrence counties.

Compensation starts at $60,000 annually with comprehensive benefits. requires MSW, state licensure, and passion for patient-centered renal care. New MSW grads welcome; renal care experience preferred.

Qualifications

• MSW degree required and licensed to practice in the state.
• Experience in renal or dialysis settings is a plus but not required.
• Strong communication, assessment and advocacy skills.

Responsibilities

• Address psychosocial needs of dialysis patients to improve outcomes.
• Assist patients with community resources for nutrition, transportation and housing.
• Advocate for access to healthcare benefits and services.
• Educate patients and families on ESRD treatment options.
• Coordinate translation services for patients and caregivers.
• Develop individualized care plans with the care team.
• Support quality improvement initiatives through data-driven efforts.
• Collaborate with interdisciplinary staff to adjust care plans.
