Qualifications

• Master's degree in Social Work required.
• Must have state required license.
• Meets licensure requirements in the state of employment.

Responsibilities

• Assess patients' psychosocial status and participate in care planning to support rehabilitation and goals.
• Provide supportive counseling and patient education to patients and families.
• Collaborate with interdisciplinary team to address psychosocial needs and barriers to treatment.
• Provide information and assist with transplant referrals and waitlist management.
• Connect patients with community resources and vocational/educational supports as needed.
• Maintain knowledge of patient rights, advance directives, and policy requirements.
