Job summary

Fresenius Medical Care is seeking an entry-level Registered Nurse for the Outpatient CAP program. You will provide and coordinate clinically competent care for an assigned patient group undergoing hemodialysis, including assessment, planning, intervention and evaluation.

As part of the End Stage Kidney Disease care team, you will participate in teaching, leadership and quality improvement activities and may delegate tasks to RNs, LVNs/LPNs and Patient Care Technicians as needed.

Qualifications

• Entry level RN with less than 2 years of nephrology nursing experience.
• Chronic/acute hemodialysis experience preferred.

Responsibilities

• Coordinate and provide clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, PCTs) when needed.
• Document data collection pre-, during-, and post-hemodialysis in the medical record; adjust care plan as indicated.
• Educate patients and families on learning needs; provide ongoing education.
• Direct daily provision of safe, effective hemodialysis therapy in collaboration with care staff.
• Administer medications as prescribed and document effectiveness.
• Initiate or assist with emergency response measures.
• Serve as resource and participate in staff training and orientation.
• Ensure proper lab collection, processing and timely rescheduling as needed.
• Update nursing assessments and care plans with Interdisciplinary Team.
