The nurse conducts assessment, planning, intervention, and evaluation, participating in quality improvement and patient education initiatives within an ESKD care team.

Qualifikationen

• Graduate of an accredited School of Nursing.
• Current state licensure to practice nursing.
• CPR BLS Certification.
• Minimum 9 months RN experience; 3 months dialysis experience preferred.

Aufgaben

• Performs essential nursing functions under supervision with guidance from Clinical Educator, Preceptor, or RN leadership.
• Collects and analyzes patient data during dialysis and documents in medical record; adjusts care plan as needed.
• Educates patients and families on learning needs and treatment options.
• Delivers safe and effective hemodialysis therapy in collaboration with care team.
• Administers medications per protocol and documents justification/effectiveness.
• Initiates or assists with emergency responses when needed.
• Serves as a resource and participates in staff training and orientation.
• Ensures proper lab collection, processing, and scheduling of collections.
• Collaborates with Interdisciplinary Team to update nursing assessments and plans of care.
• Addresses transplant and treatment modality options with patients.
