Job summary

Creative Solutions Services, LLC is seeking a Dialysis Chronic Care Registered Nurse (RN) for Hampton, VA. The role focuses on the provision and coordination of dialysis care, including assessment, planning, intervention, and evaluation for assigned patients.

The RN will work within the End Stage Kidney Disease team, collaborating with RNs, LVN/LPNs, and patient care technicians to ensure safe, effective hemodialysis therapy and quality outcomes.

Qualifications

• Registered Nurse with dialysis experience required.
• 12 months or more of recent dialysis experience.
• Current CPR/BLS certification.

Responsibilities

• Provide and coordinate clinically competent care for assigned patients.
• Assess, plan, intervene, and evaluate patient care with the care team.
• Administer medications as prescribed and respond to emergencies.
• Educate patients and families about care plans and treatment options.
• Document in patient records and communicate with the interdisciplinary team.
