Qualifications

• High School diploma or GED required.
• CMS/state certification requirements within the required state or CMS timeline.
• All appropriate state licensure, education, and training (if any) required.

Responsibilities

• Assist other health care members in patient education regarding adequacy measures (OLC, AMP, UKM).
• Evaluate vascular access for patency and perform cannulation and heparin administration as delegated.
• Obtain pre- and post-treatment vital signs and weight; evaluate pre-treatment access.
• Initiate dialysis per orders including QB and QD.
• Monitor patient response to dialysis and document treatments.
• Ensure accurate documentation of Hemodialysis Treatment Sheet and progress notes.
• Maintain infection control practices and prepare laboratory specimens as required.
• Prepare initial patient charts for admission and organize lab requisitions
