Job summary

Qualifications

• MSW from CSWE-accredited program; LCSW preferred.
• Current licensure in applicable state required unless AZ/PA or Guam exception.
• Experience providing social services to dialysis patients preferred.
• Strong written and verbal communication skills; computer proficiency.
• Ability to participate in Interdisciplinary team and care planning.

Responsibilities

• Provide social services to patients and families in dialysis/rehabilitation.
• Identify psychosocial issues and counsel patients and families.
• Assist with program goals and patient outcomes in quality care.
• Collaborate with Medical Director and physicians.
• Complete KDQOL and participate in QAPI/meetings.
