Qualifications

Job description

Diabetes Registered Nurse Educator, Endocrinology Clinic, Days, Full-Time(Endocrinology Clinic)

The Registered Nurse Diabetes Educator plays a critical role in supporting patients with diabetes and related endocrine disorders. This position provides individualized diabetes education, coordinates care, and empowers patients with the knowledge and tools needed for effective self-management. Working as part of the endocrinology team, the RN Diabetes Educator ensures delivery of evidence-based care and supports quality improvement efforts related to diabetes outcomes.

Essential Duties and Responsibilities:
  • Provide one-on-one and group diabetes education to patients and families, including newly diagnosed individuals and those requiring ongoing support.
  • Develop and implement individualized care plans in collaboration with providers and multidisciplinary teams.
  • Educate patients on insulin administration, blood glucose monitoring, nutrition, lifestyle changes, and use of technology (e.g., CGMs, insulin pumps).
  • Participate in the assessment, planning, implementation, and evaluation of care.
  • Maintain accurate documentation of educational sessions and patient progress in the electronic medical record (EMR).
  • Act as a clinical resource for staff and patients regarding diabetes-related topics.
  • Assist with quality improvement initiatives focused on diabetes management and clinical outcomes.
  • Maintain knowledge of the latest advancements in diabetes care and education.
  • Comply with all policies and procedures related to safety, infection control, and patient privacy.
Required Qualifications:
  • Active Registered Nurse (RN) license in the state of Kentucky.
  • Minimum 2 years of experience working with patients with diabetes.
  • Strong understanding of diabetes pathophysiology, medications, and self-management principles.
  • Excellent communication, teaching, and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Proficient with EMR systems and Microsoft Office Suite.
Preferred Certifications (Strongly Preferred):
  • Licensed Diabetes Educator (LDE)
  • Certified Diabetes Care and Education Specialist (CDCES) (formerly CDE)
  • Certification or affiliation with ADCES (Association of Diabetes Care & Education Specialists)
  • ? If not currently certified, candidate must be willing to obtain one of the above certifications within 12 months of hire
Certification Requirements (All)

BLS
ACLS
KY licensed RN
LDE, ADCES, or CBDCE within 6-12 months
