Job summary

St. Luke's University Health Network is seeking a Diabetes/Patient Educator, Maternal Fetal Medicine who will provide comprehensive nutrition care to patients through assessments, diet counseling and diabetes support and education programs.

The role requires collaboration with a Perinatologist and Perinatal Nurse Practitioner, development of individualized learning plans, and the use of ADCES/AADE7 outcomes framework to measure effectiveness. Must be able to travel to other MFM locations.

Qualifications

• Registered Nurse or Registered Dietitian with a Bachelor of Science degree and current license to practice in Pennsylvania.
• Certification as a Diabetes Educator preferred.
• ADA/CDCES certification as a Diabetes Educator within 2 years.

Responsibilities

• Develops education goals, learning objectives and individualized plans for diabetes and pregnancy education.
• Collects assessment data using the ADCES framework and tracks outcomes.
• Assesses patients’ glucose levels and plans care with Perinatologist and Perinatal Nurse Practitioner.
• Utilizes Perinatal Diabetes and Pregnancy management protocols.
