Job summary

St. Luke's Health Network, Inc. is seeking a Diabetes/Patient Educator for Maternal Fetal Medicine. The role provides comprehensive nutritional care, diabetes support, and education to patients, with emphasis on collaborative planning and outcome-driven education.

The educator will utilize ADA standards, employ interactive teaching methods, and development of individualized follow-up plans in coordination with perinatal specialists. ADA/CDCES certification within 2 years is preferred.

Qualifications

• Registered Nurse or Registered Dietitian with BS and current PA license.
• ADA/CDCES certification as a Diabetes Educator preferred or eligible within 2 years.

Responsibilities

• Collaboratively develops education goals, learning objectives and personalized plans for diabetes and pregnancy education.
• Collects diabetes and pregnancy program assessment data using ADA/AADE frameworks.
• Assesses patients' blood glucose and develops care plans with Perinatologist and Perinatal NP.
• Familiar with Perinatal Diabetes and Pregnancy management protocols and ADA Standards of Care.
• Uses interactive, skill-based training methods and creates follow-up plans.
• Documents education plans, services and evaluation results in patient records.
• Collaborates with the care team and communicates effectively with team members.
