Job summary

The Eastern Shoshone Tribe is seeking a Diabetes Outreach Worker to enroll patients with diabetes and at‑risk individuals in Diabetes Self‑Management Education (DSME) programs. You will facilitate classes, assist with home visits, transportation, and coordinating care with Tribal Health services.

Responsibilities include conducting screenings, collecting health data, providing prevention presentations, and maintaining program records under the Community Health Coordinator’s guidance.

Qualifications

• Excellent communication, planning, problem solving and computer skills.
• Ability to work with a team and with community members of all ages.
• Knowledge of disease prevention, intervention and health education with HIPAA compliance.

Responsibilities

• Conduct and collect data for screenings, home visits, and self-management classes.
• Collect data for prevention, BMIs, and physical activities.
• Conduct prevention presentations.
• Coordinate and collaborate with other programs for diabetes prevention events.
• Work under supervision of the Community Health Coordinator and assist co-workers as needed.
• Maintain CHR and Diabetes Program files and submit monthly reports.
