Job summary

Cochise County in Bisbee seeks a Community Health Worker (Diabetes Prevention and Education) to provide community-based outreach and education on diabetes prevention and self-management, under grant guidelines.

Requires a high school diploma, two years in community service, valid AZ driver’s license; CHW Core Competency training within six months; duties include screenings, referrals, and collaboration with partners. This grant-funded role is supported through May 31, 2031.

Qualifications

• High School diploma or equivalent and two years of experience serving in a community service role.
• Must possess and maintain a valid Arizona Driver’s License and the ability to be covered by county insurance.
• Community Health Worker Core Competency training within six months of hire; program-specific training before independent duties.

Responsibilities

• Conducts community outreach and engages eligible participants in diabetes prevention and self-management services.
• Provides individual and group diabetes prevention, self-management, nutrition, physical activity, and healthy lifestyle education.
• Performs screenings and point-of-care testing following established protocols and training.
• Identifies participant needs and connects to healthcare and community resources; initiates referrals and follow-up.
