Qualifications

• Experience in providing diabetes education and nutrition counseling.
• Ability to coordinate patient education with healthcare teams.

Responsibilities

• Coordinates patient and support system assessment, education, planning and implementation of care goals.
• Collaborates with referring providers and interdisciplinary team to improve education outcomes and glycemic control.
• Maintains CDCES standards in an ADA approved Diabetes Self-Management Education program.
• Plans and conducts nutritional programs under supervision to promote health and control disease.
• Assesses nutritional needs considering cultural, educational, and learning factors.
• Provides individualized nutrition education to patients and families based on needs assessment.
• Serves as a nutrition resource to nurses and other health care providers.
• Communicates patient information to providers and staff in a timely manner.
• Documents education discussions, materials provided, and patient comprehension.
• Performs other related duties as assigned.
• Position is at two locations, with two days per week at 435 South Street, Morristown and two days per week at the Rockaway Pavilion.
