• Coordinates individualized patient and support system assessment, education, planning and implementation of care goals.
• Collaborates with the referring providers and interdisciplinary team to work toward optimal patient education outcomes and glycemic control.
• Maintains professional education standards required to be a CDE in a ADA approved Diabetes Self- Management Education program.
• Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.
• Assesses nutritional needs and considers patients' cultural, educational, and learning issues.
• Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment.
• Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
• Communicates patient information to referring providers and appropriate staff in a timely manner.
• Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.
• Performs other related duties as assigned.
