Job summary

Atlantic Health in Morristown, NJ seeks a Diabetes Educator/Registered Dietitian to coordinate patient education and nutrition planning. You will collaborate with referring providers and an interdisciplinary team to optimize glycemic control and education outcomes.

This role requires maintaining CDE credentials and delivering culturally sensitive nutrition education, with duties across various patient education scenarios within the facility.

Qualifications

• Must hold CDE credential recognized by ADA.
• Licensed or eligible to practice as a dietitian in NJ.
• Experience delivering diabetes self-management education.

Responsibilities

• Coordinates individualized patient and support system assessment, education, planning and implementation of care goals.
• Collaborates with referring providers and interdisciplinary team to work toward optimal patient education outcomes and glycemic control.
• Maintains professional education standards required to be a CDE in a ADA approved Diabetes Self-Management Education program.
• Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.
• Assesses nutritional needs and considers patients' cultural, educational, and learning issues.
• Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment.
• Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
• Communicates patient information to referring providers and appropriate staff in a timely manner.
• Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.
• Performs other related duties as assigned.
