Job summary

Bryan Health in Lincoln, NE seeks a Diabetes Educator to provide expert education and self-management training for patients with diabetes across all ages. You will assess learning barriers, set goals, implement evidence-based interventions, and measure outcomes within a comprehensive glycemic management program.

In this role you will liaise with physicians and the care team, maintain up-to-date education materials, and coach staff to deliver best-practice diabetes education consistent with ADA

Qualifications

• Current Registered Nurse licensure and minimum 2 years clinical practice required.
• Bachelor's in Dietetics or RD with LMNT and 2+ years clinical experience required; CDE preferred.
• CDE certification preferred within organization’s guidelines.

Responsibilities

• Assess patient needs by identifying barriers to learning and preferred learning methods.
• Develop and implement an education plan and document actions per policy.
• Collaborate with interdisciplinary team to achieve patient and program goals.
• Incorporate cultural considerations into care delivery and education.
• Provide diabetes education across all age groups per ADA standards and program requirements.
• Maintain state-of-the-art diabetes care and ongoing program evaluation.
