Qualifications

• DSMES program participant assessment and education planning

Responsibilities

• Instruct DSMES program participants
• Collect assessment data and complete participant evaluations
• Develop individualized educational plans with goals and outcomes
• Provide DSMES interventions using interactive, patient-centered coaching
• Create follow-up DSMES plans and monitor progress
• Evaluate effectiveness of education against goals
• Assist in curriculum development and training materials as needed
• Participate in quality improvement activities for the DSMES program
• Support provider marketing efforts to sustain referrals
