Job summary

Allegheny Health Network is seeking a Diabetes Educator to assess patient need and deliver individualized self-management education. The role includes planning continuing education for staff and the community, and documenting outcomes in each participant's record.

The ideal candidate holds a Bachelor's degree and is a registered/licensed Dietitian with experience counseling patients with diabetes and delivering community education. A Master’s degree and CPT are preferred but not required.

Qualifications

• Bachelor’s degree or higher in a health-related field.
• Registered or licensed Dietitian with current credentials.
• Experience counseling patients with diabetes and providing community education.

Responsibilities

• Collect DSME/T program participant assessment data using a recognized certification database.
• Develop educational goals, learning objectives, and a content plan for participants.
• Create individualized follow-up plans and evaluate outcomes against learning objectives.
• Conduct follow-up assessments using AADE7 outcome measures.
• Document assessments, plans, services and results in participant records.
• Contribute to development of education curricula and materials.
• Perform other duties as assigned.
