PeaceHealth in Longview, WA is seeking a Part-time Diabetes Educator to support patients through individualized or group diabetes education in a hybrid setting. The position offers three 8-hour shifts per week with potential for partial remote work.

You will provide diabetes self-management training, develop follow-up plans, and document care in the medical record. Excellent communication and teaching skills are essential for success in this role.

Qualificações

• Bachelor’s in Dietetics or related field required for Dietitians
• Registered Dietitian or WA-licensed RN with the CDCES credential within 12 months
• CDCES certification within 12 months of hire
• Minimum 1 year experience in dietetics with chronic conditions including diabetes, CV, renal disease
• Ability to teach effectively in group settings and use MS Office tools

Responsabilidades

• Develop educational goals and plans for program participants
• Provide diabetes self-management training in clinic or hospital settings
• Create individualized follow-up plans for patients
• Document patient care in medical records per standards
• Contribute to training materials and quality improvement processes
• Assess access to diabetes medications and testing supplies
• Keep education materials up to date with guidelines
• Perform other duties as assigned
