• Experience with one-on-one and group education; community health experience preferred.

Responsibilities

• Provide nutrition and diabetes education to patients and community members in one-on-one and group settings.
• Assess nutritional needs and develop and implement nutrition care plans.
• Evaluate and report outcomes of nutrition interventions to care teams.
• Maintain competence in basic aspects of the job within onboarding period.
