Job summary

Froedtert Health in Manitowoc is seeking a part-time Diabetes Educator to perform in-depth diabetes assessments, develop plans of care, and provide outpatient education to patients and the community.

The role requires Wisconsin RN licensure (or RD/CDE credentials), strong teaching skills, and the ability to work independently as part of a care team. BLS certification is expected, with opportunities for professional development.

Qualifications

• Graduate from an accredited school of nursing with current license to practice in the State of Wisconsin required.
• Registered Dietician also acceptable.
• Certified Diabetic Educator (CDE) certification preferred.

Responsibilities

• Performs in-depth assessments and formulation of individual plans of care for people with diabetes.

Location: US:WI:MANITOWOC at our NE-HARBOR TOWN CAMPUS facility.

Job Type: On-site; FTE: 0.5; Standard Hours: 20.00; Shift: Shift 1.

Job Summary

Performs in-depth assessments and the formulation of individual plans of care for people with diabetes. Acts as a resource to professional staff and the community, providing outpatient diabetes education. Participates in hospital and community in‑services and support groups. Investigates grant funding for projects associated with diabetes and chronic conditions associated with diabetes.

Education

Graduate from an accredited school of nursing with current license to practice in the State of Wisconsin required. Registered Dietician also acceptable. Certified Diabetic Educator (CDE) certification preferred.

Experience

Minimum of three years of experience in field of expertise preferred.

Special Skills

Demonstrates understanding of teaching/learning process, nursing theory and practice. Excellent communication and understanding of teaching/learning process, nursing theory and practice. Must be well organized, flexible, and able to work independently as well as within a team of professionals.

Licensure

BLS certification required.
