Job summary

The HUB Diabetes Care and Prevention Navigator is a grant-funded, part-time CHW role serving Iowa communities, including rural areas, in coordination with MyHealthIowa.

The Navigator conducts screenings, assesses needs, provides education, and supports referrals to diabetes prevention and management programs, food and nutrition resources, medical homes, and survivorship services, while documenting activities for grant reporting.

Qualifications

• Minimum 18 years old.
• High school diploma or GED; health field preferred.
• Bilingual English and Spanish required.
• Experience with community outreach and health screenings preferred.
• Ability to travel to community sites for events.

Responsibilities

• Support community screenings with participant navigation and follow-up.
• Educate participants about diabetes prevention, nutrition, and physical activity.
• Coordinate referrals to National DPP, DSMES, and related services.
• Maintain accurate data entry and reporting for grant requirements.
• Collaborate with partners and providers to strengthen referral pathways.
