Job summary

iCIMS, Inc. is seeking a Community Health Representative under the Community and Social Services Director to fulfill objectives tied to the Diabetes Grant, Prop 63, ICWA, and Title V Self-Governance.

You will work within the Community Services Department to support tribal health initiatives. Responsibilities include health education, home visits, transportation for appointments, patient care coordination, and data reporting.

Qualifications

• Requires High School Diploma or GED and a healthcare-related Bachelor’s degree or equivalent.

Responsibilities

• Provides health education, home visits and testing, transportation of patients for appointments, and any medical recommendations at the direction of the Medical Department.

Education

High School Diploma or GED

Bachelor’s Degree in Healthcare field

CPR Certification

First Aid Certification

Three years healthcare experience

Basic computer proficiency (MS Office)
