Job summary

Qualifications

• Graduation from high school or equivalent; six months of experience with local, state, and federal assistance programs including health care and social service systems.
• Equivalent combination of training and experience that provides necessary knowledge, abilities, and skills.

• Support patients as they transition from emergency care to ongoing education, services, and community support.
• Assist clients with understanding their conditions and accessing health insurance and medications.
• Coordinate with team for follow-up, advocacy, and home visitation as needed.
