Job summary

WVU Medicine in West Virginia is seeking a Diabetes Education Specialist RN to assess patients, deliver education, and support diabetes management across care settings. You will work with a multidisciplinary team, participate in community programs, and help maintain ADA certification.

The role requires active nursing licensure, CDCES certification within two years, and BLS; you will also typically have BSN and 2+ years nursing experience including diabetes care.

Qualifications

• Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
• CDCES Certification within two years of hire date.
• BLS Certification within 30 days of hire date.
• Two years of clinical experience, with one year in diabetes care.

Responsibilities

• Identifies education needs of the community and those with diabetes.
• Attends diabetes advisory committee meetings as assigned.
• Performs patient assessment, delivers education and evaluates patient’s obtainment of goals.
• Assists in maintaining the American Diabetes Association certification as applicable.
• Participates in community outreach and educational support.
• Advocates on behalf of the patient and families.
• Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.
• Participates in performance improvement activities to assure quality patient care.
• Completes daily rounds to assess patient status.
• Ensures all necessary testing and scheduling is complete prior to necessary procedures.
• Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
• Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program.
