EmblemHealth seeks a skilled diabetes care manager to deliver population-based care coordination, DSME, and nutrition guidance across a high-risk cohort. You will engage members by phone and support virtual education sessions to improve outcomes and close care gaps.

The role emphasizes A1c reduction, patient engagement, and collaboration with clinicians, ensuring culturally competent education and effective utilization of community resources.

Qualifications

• Active RDN license in NY (compact preferred).
• CDCES certification required for this role.
• 4–6+ years of clinical experience, including 3+ years in diabetes care or care management.
• Experience in managed care, care coordination, or population health is a plus.
• Experience with telehealth and electronic medical records systems is required.

Responsibilities

• Manage a high-volume panel of complex, high-risk members in the diabetes program.
• Conduct telephonic assessments to identify needs and gaps in care.
• Develop and evaluate individualized care plans focused on A1c improvement and risk reduction.
• Perform ongoing outreach and engagement to ensure continuity of care.
• Close gaps in care such as A1c monitoring and preventive screenings.
• Coordinate services across providers and internal teams for safe, timely care.
• Deliver DSME/T using ADCES7 framework and motivational interviewing.
