Job summary

HealthPartners Lakeview Hospital seeks a Lead Diabetes Care and Education Specialist to guide the Diabetes Care and Education Service Line. You will lead daily activities, support colleagues, and promote high-quality patient education and care for individuals with diabetes.

The role requires RN or RD credentials with diabetes expertise, leadership skills, and Minnesota licensure, plus BLS and CDCES eligibility. Expect a collaborative, patient-focused team environment.

Qualifications

• Requires RN or RD with advanced knowledge in diabetes care.
• Leadership experience in a clinical diabetes program is preferred.
• Licenses and certifications must be active in Minnesota at hire.

Responsibilities

• Provide direct care, education, and support across the lifespan for people with diabetes.
• Promote self-management and improve health outcomes using evidence-based strategies.
• Collaborate with managing clinicians to deliver integrated, person-centered care.
• Lead daily work activities within the Diabetes Care and Education Service Line.

Bachelor’s degree in nursing or nutrition and/or Dietetics with 2 years of diabetes care and education experience

Associate’s degree in nursing with 4 years of diabetes care and education experience

Minnesota RN licensure or Minnesota Dietitian licensure

Current CDCES certification or ability to become certified within 24 months of hire

Current BLS certification for health professionals
