Northwestern Community Services Board is seeking a DD Case Manager in Virginia to assess needs, develop service plans, and connect clients with supports across providers. You’ll maintain records, liaise with agencies, and advocate for services while ensuring compliance with Medicaid and licensure guidelines.
The role requires a relevant human services degree, a valid driver’s license, and the ability to travel within the catchment area to visit clients as needed.
The Developmental Disability (DD) Case Manager serves to assist in the provision of services to developmentally disabled individuals of the jurisdiction(s) served by his/her clinic site position location. Primary duties include individual assessment, service plan development, residential placement and follow-up, information and referral, case recording, and liaison with other agencies and facilities which serve developmentally disabled individuals.
