Northwestern Community Services Board is seeking a Developmental Disabilities Services Intake Specialist to coordinate the assessment and intake process for individuals with developmental disabilities and their families in Virginia.
You will build partnerships with community resources, conduct eligibility assessments, and maintain records in the Virginia Waiver Management System to support DD waiver eligibility and access to services.
The Developmental Disabilities Services Intake Specialist is responsible for coordinating the assessment and intake process to individuals with developmental disabilities and their families while ensuring compliance with state and Agency guidelines. This role involves developing strong relationships with community partners, conducting comprehensive eligibility assessments, and providing support and resources for individuals on the DD waiver waitlist. Key responsibilities include evaluating individuals for diagnostic and functional eligibility, determining priority levels, and maintaining accurate records in the Virginia Waiver Management System. The position also requires ongoing engagement with individuals and families, offering education, resources, and crisis intervention as needed. This role involves collaboration with internal and external stakeholders to address service gaps, facilitate referrals for case management, and provide excellent customer service. Additional duties include outreach activities and serving as a liaison on the Project Search steering committee.
