  • Apply knowledge of early childhood development to deliver services.
  • Conduct face-to-face family intakes and needs assessments.
  • Engage with families to access developmental and behavioral services for their children.
  • Collaborate with community agencies to coordinate care for patients.
  • Provide case management and coordination services to families.
  • Develop partnerships with external agencies and connect families to services.
  • Track and follow up external referrals.
  • Partner with Health Education to deliver prenatal workshops and early literacy programs.
  • Educate families about advocacy for their children.
  • Advocate with service providers to ensure timely and appropriate care.
  • Document clinical encounters and program information promptly.
  • Ensure the quality dashboard in follow-up of high-risk patients.

Salary Range: $34.37 - $38.85 Hourly

SUMMARY OF POSITION:

NEMS is looking for a Development Behavioral Health Program Coordinator to support the healthy development of children. They will be part of the team-based approach to improve developmental and behavioral health. Pediatricians, Social Workers, and Family Engagement Specialists partner with families to ensure children have access to universal screening and early intervention services. This role is a critical member of the pediatric team, focusing on (1) engaging parents, (2) coordinating care, (3) providing small group workshops on baby-bonding and language development (4) connecting families to services, (5) advocating for families, and (6) supporting optimal growth and development of children.

ESSENTIAL JOB FUNCTIONS:
  • Apply knowledge of early childhood development, effective school readiness strategies, and positive parenting skills to effectively deliver services
  • Conduct face-to-face family intakes and needs assessments
  • Engage with and support families in accessing developmental and behavioral services for their children.
  • Engage collaboratively with community-based agencies to assure continuity and coordination of care for patients.
  • Provide case management and coordination services to families.
  • Develop and support partnerships with external agencies and connect families to their services.
  • Track and follow up external referrals.
  • Partner with the Health Education team to deliver prenatal workshops and early literacy programs.
  • Educate families about advocacy for their children.
  • Advocate with service provider agencies to ensure timely and appropriate care for patients.
  • Document clinical encounters and program information appropriately and in a timely manner.
  • Ensure the quality dashboard in the follow-up of high-risk patients for the JVH Program.
  • Perform other job duties as required by manager/supervisor.
QualificationsQUALIFICATIONS:
  • Bachelor’s degree in Child Development, Education, Social Work, or related field required
  • Two (2) + years of experience working with children and family services preferred
  • Effective communication skills
  • Current documentation of Basic Life Support is required.
LANGUAGE:
  • Must be able to fluently speak, read and write English.
  • Fluent in Chinese (Cantonese and/or Mandarin) required, Vietnamese, Spanish and other languages may be required depending on location.
  • Fluency in other languages are an asset.
STATUS:

This is an FLSA Non-exempt position.

This is not an OSHA high-risk position.
