Qualifikationen

• RN licensure preferred; or bachelor’s degree in health or human services with CCM, CDMS, or CRC.
• Valid driver’s license required.

• Support recovery and autonomy through advocacy, communication, and education.
• Identify providers and facilities to ensure timely, cost-effective care for the client and reimbursement source.
