Job summary

28th Street Detox LLC in San Diego, CA is seeking a dependable LVN for a full-time day shift to support detoxification and residential treatment. You will perform nursing duties, document accurately, and work with an interdisciplinary team.

The role requires weekend availability, strong clinical judgment, medication management, and proficiency with EMR/EHR systems. California LVN license, First Aid/CPR, and prior detox or behavioral health experience are preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active, and unencumbered California LVN license.
• Ability to work a full-time day-shift schedule, including weekends.
• Current First Aid and CPR certification.
• Knowledge of nursing practices, medication administration, documentation standards, and applicable healthcare regulations.
• Ability to recognize and communicate changes in a client\'s medical or behavioral condition.
• Ability to prioritize responsibilities and work effectively with minimal supervision.
• Strong written and verbal communication skills.
• Computer proficiency and experience using EMR/EHR systems.
• Ability to work independently while functioning effectively as part of an interdisciplinary team.
• Strong attention to detail and commitment to timely, accurate clinical documentation.

Responsibilities

• Complete nursing duties and documentation in accordance with facility policies.
• Complete and document client rounds as required.
• Conduct medication and controlled-substance counts.
• Review the shift log and complete required documentation.
• Complete nursing admission procedures and assessments.
• Document nursing progress notes and documentation in the EMR/EHR.
• Observe and document medication administration.
• Obtain vital signs and monitor client status.
• Complete daily nursing assessments for detoxification clients.
• Conduct CIWA-Ar, COWS, and polysubstance withdrawal assessments as ordered.
• Monitor changes in withdrawal symptoms and medical/mental status.
• Communicate changes to physicians and other providers.
• Provide medication education and reinforce compliance.
• Coordinate medication refills and pharmacy needs.
• Provide crisis intervention as needed.
• Supervise Behavioral Health Technicians as appropriate.
