Adaptive Home Health, LLC is seeking a Certified Occupational Therapy Assistant (COTA) for field-based home health visits in the Denver area. You will deliver skilled visits under the OT plan of care, helping patients stay safe and independent while traveling across the assigned territory.

The role offers a full-time schedule with occasional weekends, supported by an upside productivity model and technology like Homecare Homebase for efficient documentation and scheduling.

Qualifikationen

• Active Denver COTA license (in good standing).
• Strong clinical judgment and comfort working independently in the field (within COTA scope and under OT supervision).
• Strong communication and documentation.
• Current driver's license and comfort with daily travel across assigned territory.

Aufgaben

• Deliver skilled COTA visits in the home under the supervising OT's plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/concerns to the supervising OT.
• Coordinate with nursing and the broader care team: escalations as needed.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
