Job summary

Canyon Colorado Home Care & Hospice in Denver, CO is seeking a Registered Nurse to administer skilled nursing care in patients' homes. The role follows physician orders and a plan of care under supervision of the Clinical Manager, with a night schedule of 7 nights on 5pm-8:30am and weekends as outlined.

The position requires nursing licensure in the state, preferably 1 year of nursing experience, and alignment with the agency’s philosophy.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed.
• One year nursing experience preferred.
• Acceptance of philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit, determines the patient’s needs, and develops the plan of care.
• Regularly reevaluates nursing needs and revises the care plan accordingly.
• Initiates the plan of care and necessary revisions.
• Furnishes services requiring specialized nursing skill.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences.
• Coordinates services with IDT members, physicians, other providers, patient and caregivers.
• Informs physician of changes in patient condition and needs.
• Counsels patient and family in meeting nursing-related needs.
• Participates in staff meetings, peer review and quality management as assigned.
• Completes and submits OASIS assessments and discharge summaries.
• Observes confidentiality and adheres to Agency policies.
